Services

Homemaker/Personal Care
Includes a variety of services and supports necessary for the health and welfare of an individual, which enables the individual to live in the community.

May include activities of daily living, personal hygiene, dressing, feeding, transfer, and ambulatory needs or skill development.



Remote Monitoring
Full-Service remote monitoring provider capable of providing Remote Monitoring equipment as well as monitoring and backup staff as defined under the IO Waiver.  See our pages for complete details on this service.



Transportation
Transporting the individual served to any community locations. This service is provided for maximum community and social integration, as well as ensuring all appropriate medical needs and ancillary services are being provided

Transportation may be provided in normal vehicles or specially equipped vehicles, such as wheel chair vans.



Social Work
Social work services are commonly used to provide behavior support or other advocacy services for the individual served.



Nutritional
Nutritional Services are used to develop specialized diets for the individual served, or provide other areas of nutritional counseling.



Environmental Modification
Similar to Environmental Accessibility Adaptations, this service is used to modify the individuals home to ensure it is the safest, least restrictive environment possible.



Nursing

At this time, our nursing services are utilized solely for the purposes of training our staff in all of the current DODD requirements for medication administration.  These classes are also available to non-employees.



Interpreter
Interpreter services may be used to assess an individuals communication ability, and to work as a link between the individual served and their home staff.

Interpreter services are commonly used to train staff to effectively communicate with the individual.



Adaptive & Assistive Equipment
Individuals served under the appropriate waiver may use this service to purchase items used to adapt them to their environment. In an attempt to promote as much independence and freedom possible, items such as specialized cups or utensils, grabbing devices, etc. may be purchased to eliminate common barriers to independence.



Supported Employment
Supported employment services may be utilized to qualified individuals to assist them in obtaining and maintaining employment in the community.

This service may help the individual find suitable community employment, and provide job training to assist in the retention of the job.

Environmental Accessibility Adaptations
Involves the construction of environmental adaptations to provide the maximum amount of accessibility and mobility to the individual in their home environment.

May include the construction of wheelchair ramps, bathroom or kitchen modifications, or other modifications of the home in increase accessibility.



Specialized Medical Equipment & Supplies
Includes the purchase of specialized medical equipment with the intent to meet the specific needs of the individual served.

Such equipment may include wheel chairs, or wheel chair accessories, adaptive eating utensils, electronic devices, etc.



Transportation

Personal Emergency Response Systems
Personal emergency response systems may be designed and installed to meet the needs of the individual served.

The emergency response systems may be developed to monitor the location of the person served, or provide a link to all necessary emergency services.
